Job Description

Saint Alphonsus is looking for Vascular Surgery ongoing call coverage starting in July 2023!
Shift(s) Description:

• 24 hour call coverage, 7a-7a

Coverage Needed: 

• 7/7-7/10
• 8/11-8/14
• 9/1-9/5
• 11/23-11/27
• 12/22-12/26

Details:

• Locums will NOT be responsible for supervising other providers
• Ongoing coverage needed will be sporadic weekends and holiday coverage
• EMR: EPIC
• Level II Trauma- HIGH ACUITY and INTENSITY 
• Client will reimburse for travel

Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process:

• MD diploma
• Internship/residency/fellowship certs
• All medical licenses- COVID-Related Physician Licensure Exemption - Idaho Code 54-1804(1)(d)
• DEA
• Board Certification
• Life support cards- BLS
• Current and previous certificates of insurance
• Driver’s license
• Professional color photo/headshot
• CMEs for the past 2 years
• Immunization records including TB, Flu, HepB, Tdap, Varicella, and COVID
• References
• Case logs

  SUBMISSION REQUIREMENTS

• Board Certified - Required
• Active ID license at the time of submission -Highly Preferred, will potentially consider IMLC candidates 
• DEA and CSR - Required
• BLS - Required
• Local candidates - highly preferred
• Must meet perm rate cap of $25k
• *Must submit proof of COVID vaccine in order to work at this facility - Required at time of submission** - Verbal Confirmation needed at time of name clear

CERTIFICATION REQUIREMENTS

• Board Certified
• Controlled Substance (CSR/CDS)
• DEA

STATE LICENSE REQUIREMENTS

• Idaho
