Saint Joseph is seeking a Urologist to assist with 24 hour Hospital call coverage for the specific dates listed. These are the current urgent dates that need to be filled, provider will be added to a pool for ongoing coverage.Â
Credentialing Timelines:
a.) Payer enrollment: 90-120 days from receipt of Demographics, IN license, IN Controlled Substance Registration, DEA w/IN practice address, and signed enrollment forms.Â
b.) Hospital Privileges: 45-60 days from receipt of complete CVO/CPI Portal Application.Â

- EMR: EPIC
- General Urology procedures required
- Willing to pay for full travel
