Grand Rapids Hospital is looking for a Locum Ophthalmologist!
Details:
Practice Setting: Hospital based
Reason for Coverage: Trauma
Dates of Coverage: January- July
Call Schedule: one week a month
Average Number of Patients Per Day - Hospital: 1-2 per month
Will Other Ophthalmologists Provide Coverage During Locum's Coverage Dates? Yes
What Percentage of the Practice is Neonate, Pediatric, Adolescent, Adult, Geriatric: 100% Adult/Geriatric
What Percentage of the Practice is Non-Surgical, Surgical, Laser: 50%
Documentation System/EMR: Epic
Trauma Level/Designation: Level 2
Required Clinical Areas: Non-Surgical, Surgical/Invasive
Estimated Credentialing Timeframe: 3 months Submission Requirements
- Board Certified Ophthalmologist
- MI license
- BLS/ACLS - Not required, unless performing moderate or deep sedation
