The Family Health Center is looking for 2 Family Medicine or Internal Medicine Physicians to join their team in Kalamazoo, MI. It is a federally qualified health center (FQHC) that goes beyond the call of duty to take exceptional care of our community. Through community outreach, medical & dental services, behavioral health management, on-site pharmacy, and so much more. We are there for the members of our community every step of the way.Â
Job Description:
- Accepting full-time and part-time candidates
- Must be willing and able to perform common office-based, outpatient surgical procedures (i.e. ambulatory surgery, stitches/staples, superficial burn treatment, biopsies, etc.)
- The Family Medicine Physician provides continuous quality patient-centered care to patients of all ages throughout their lifespan, following the Family Health Center's standards and practices. Family Medicine Physicians embrace a team-oriented approach in providing healthcare and be comfortable working with and overseeing Physician Assistants and providing collaboration with Nurse Practitioners.
- Provides healthcare and health management for patients of all agesÂ
- Outpatient and clinic setting
- Averages 18 patients per day on the high end
- Provides a total assessment of the patient's health status, including systematic health history, a physical examination, and diagnostic testing
- Provides skilled observations and health assessments of patients, including history, physical examination, and appropriate interventions and referrals
- Provides patient care services appropriate to patient condition and in conjunction with current Joint Commission standards
- Diagnoses and treats or appropriately refer to health problems for the patient population. Orders interpret and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
- Establishes and facilitates a plan of care related to a patient's identified problems, needs, and strengths, including medication, diet, therapeutic procedures, counseling, education, and teaching needs of patients, families, and groups.
- Assess and formulate a plan of care related to preventative, acute, and chronic needs identified by the patient and health care team in a manner that provides continuity of care. It includes coordinating care with patients' primary care physicians (PCP) when identifying and identifying an available PCP when one is not currently designated.
- Assesses patient/family learning needs and provides appropriate patient education.
- Analyzes reports and findings of tests and examination and diagnoses the condition of the patient.
- Determines and prescribes medication, dosage, and schedule, given the patient's condition and allergies. Â
- Discusses any possible side effects of medication or immunization with the patient.
- Prescribes and administers vaccinations to immunize patients from infectious diseases.
- Promotes health by advising patients about diet, hygiene, and methods for the prevention of disease.
- Patients refer to a medical specialist for consultant services when necessary for the patient's well-being.
- Documents the patient's visit, including medical history, physical exam, diagnoses, and plan of action
- Follows up with patient regarding progress in high risk or emergency cases
- Conducts physical examinations to provide the information needed for admission to the school, consideration for jobs, or eligibility for insurance coverage.
- Participate in all required departmental and organizational meetings. Â

- Demonstrated the ability to assess clinical needs and work in an integrated, holistic approach to care, including case management, disease management and/or population health programs.
- The ability to work within a high volume of medical practice with multiple interruptions and busy work schedule and respond to urgent and emergent situations.
- Knowledge of managed healthcare systems, medical quality assurance and quality improvement, and risk management,
- Proven excellence working in an interdisciplinary collaborative team environment.
- Effective interpersonal skills, leadership skills, and clinical expertise in inpatient care
- Desired start date:Â 60 to 90 days after offer

- Clinicians of the Family Health Center have the opportunity to apply for up to $50,000 in loan repayment through the National Health Service Loan Repayment Program.Â

Since Moses L. Walker and his colleagues opened the doors for the first time in 1971, Family Health Center (FHC) has been serving community members who are in need of quality and compassionate healthcare. From humble beginnings in a converted trailer to the current state-of-the-art $10 million facility on the same plot of land at the corner of Paterson and Burdick Streets, FHC has become the county’s only Federally Qualified Health Center (FQHC). FHC has also been accredited as a community-based health center by the National Committee for Quality Assurance for over a decade.
What began as a group of dedicated community advocates, volunteer doctors, nurses and assistants is now, more than 53 years later, one of Michigan’s most highly recognized FQHCs. Our team offers comprehensive health services at three locations, with an additional set of mobile health and dental units. Our staff administers approximately 165,000 patient visits annually as part of our commitment to serve the community members of Kalamazoo.
Our leadership and staff at FHC are also passionate about community engagement. Our annual events include the Dr. Lisandra Soto Dental Day of Caring and Back to School Bash. FHC also provides Medical Assistant training to community members interested in pursuing this career path.
