Family Medicine Physician for Chino Valley, AZ (Locum)
Job Description
Outpatient Internal/Family Practice Physicians.Â
·      Has excellent clinical skills.
·      Is dedicated to delivering high quality, compassionate care to patients.
·      Is licensed to practice medicine in the state of Arizona.
·      Is board certified or board eligible in their specialty.
·      Demonstrates a work history of positive collegial relations with colleagues, support staff, hospital-based providers, administrators and patients.
·      Demonstrates advanced competency in their specialty.

Schedule/Availability Requirements:
Mon, Tue, Thur, Fri 7:30-5:30
Facility Location: 474 N Highway 89, Chino Valley
Setting: Outpatient ClinicÂ
Duties: Family Practice
Days per month: 20
Required Procedures: toenail removals, lesion removals, Pap Smears, Joint injections. (negotiable)
EMR: Cerner
Support Staff: Medical Assistant/Triage RN/Front Office Staff/Clinic Manager
Reason for Coverage: Provider retirement
Must be willing to do controlled substances for our Pain patients

Admissions per day, Patients/day, Types of cases, Disclosures that may need to be made, etc. No Hospital Admissions-18-20 patients per day.Â
 Requirements
- Board Certification – Board Certified or Board Eligible
- State License – AZ, IMLCÂ
- Residency Requirements/Fellowship
- Other Certifications – BLS/DEA
- Malpractice – Clean Preferred
