Reason for Coverage: Provider currently out with return date unknown.
Practice Setting: Clinic located inside hospital, hospital rounding, call and call back
Schedule (Shifts and Hours): 24/7 coverage shared with full time and prn provider
Call Ratio: needing 10-14 days call coverage per month
Average Number of Cases Per Day: 1-2
Trauma Level/Designation: 2
Number of Operating Rooms: 12 with hybrid
Documentation System/EMR: EPIC
Description of Ancillary Staff: Support staff includs 4 PA's
Patient Age Demographic (Pediatric, Adolescent, Adult, Geriatric): Adult
