Job Description
- Order Description:
- Type: PERM
- Position: CLINIC
- MD/DO FP
- MD/DO FP w/ OB
- MD/DO FP w/ OB & C-Section
- Role: Family Med
- Daily volume: 18-22 patients daily
- Any second coverage: No
- Shift options:
- Clinic Only: Four 10-hour shifts per week. Generally, start at 8am but we also do early and late clinic on a rotating schedule.
- Clinic + ER: Two 10-hour clinic shifts and one 24-hour ER shift
- Clinic + Hospitalist: Four 10-hour clinic shifts but rounding on patients in the morning and seeing clinic patients when done.
- OB/C-section call is 24/7 and rotates between the OB physicians. If the physician on call doesn't do C-sections, then other physicians may be scheduled as a "back-up" to that provider.
- EMR: Cerner
- Trauma level: IV
- Candidate Requirements:
- State License(s) accepted: Oregon
- DEA/CSR required?: YES
- Covid/Flu Vaccination Requirements: No, but if they don't have vaccinations, declination form needs to be on file
- Residents –Y/N and what are the details? No
- Life Certifications needed: NRP, ACLS
- Procedures logs needed or past amount of experience required and in what timeframe: No
- Other qualifications needed: N/A

Jatelyn and her husband Eric have 2 sets of twins who keep them very busy! Jatelyn enjoys being with her family and friends, attending/watching sporting events and going to the lake!
